Pre-lvl 60..
In the old PvP Honor System, players could earn ranks that gave them access to better items.

With Patch 2.0, the old system has been replaced with the new PvP Honor System and the PvP Arena System.

The Arena System still rewards players with new titles, but the new Honor System does not.

The old title and rank system has become obsolete, but players who earned ranks prior to Patch 2.0 can still display their lifetime highest rank.

This title was gained through the old pvp honor gain system. It took minimal effort to hardly any pvp to gain this title.

Today the title is gained through Rated Battlegrounds with a ranking of 1100.

With the new system, you gain these old PvP titles through ranking earned in Ranked Battlegrounds. So, yes.

My Druid from Vanilla has this title, and has continued to wear it with pride. It shall forever remain with him, alongside the private tabard I got him way back in Vanilla, it is his staple mark.

No. It's likely that he was awarded this title in vanilla. Since the achievement system didn't exist at that time, he would have been given the title as a feat of strength when he logged in, which must have been June 2012.
